Snap Health gives home-health agencies and clinicians access to a web system and mobile application designed to simplify referral and visit scheduling.

The product serves licensed physical, occupational, and speech therapists. It aims to reduce friction created by scattered referral sources, limited schedule control, multiple logins, and inconsistent post-visit documentation.

Simplifying a workflow shared by agencies and clinicians

Home-health referral work crosses organizational boundaries. Agencies need reliable coverage, while clinicians need visibility into available visits, location, schedule, compensation, and documentation requirements.
